Easing the Transition

We believe that good service does not stop after our client is placed in a long-term care facility. Adjusting to any new way of life can be difficult. That is why we offer follow up after placement to be sure your needs are being met during the first few months of transition. If continued care coordination is needed, we are there to help.

Whether or not we have assisted in long-term care placement, we are available to provide help in resolving complaints that may arise while you are a resident in a facility. You may be worried about health care status changes that may occur after a hospitalization or medication change or dietary issues.

Your concerns are important and should be addressed with the provider. We are happy to provide advice or intervention.
