Care Coordination
Determine/recommend residential options for the senior adult where medical, personal care, and social needs will be met in a physically safe environment.
Residential options include:
- personal residence
- active adult community
- adult day care
- senior apartments
- independent living
- assisted living
- Alzheimer’s and other dementia care
- skilled nursing facility
- continuing care retirement community
Monitor senior adult’s well-being through personal visitation and when appropriate, through telephone contact.
Coordinate healthcare supportive services such as:
- companion and nurse aid care
- medical transportation
- durable medical equipment supplies
Identify preferred medical, pharmaceutical, physiotherapy, and end of life services.
Act as personal liaison with physicians, pharmacists and hospitals, rehabilitative, palliative and hospice care professionals.
Support the senior adult and family by participating in initial and subsequent care plan meetings while residing in a long-term care facility.
Keep families informed of their loved ones quality of life and changing needs. This is especially helpful for families living at a distance.
